Outreach Care
Our outreach support service is offered to individuals and also undertaken by a small team of key workers
who have been chosen because of their ability to meet the person’s needs. Our Outreach service enables
people to remain in their own homes whilst still accessing the support they need to live the life they choose.
Outreach support can be for any number of hours a week and can be used to give family carers a break
from caring for a loved one. It also gives people a chance to have a change of scene with another
familiar face. It’s an excellent way for people with learning disabilities to try something new and
to develop their social skills, independence and self-confidence.
We offer Outreach Services to individuals with Learning Disability, Autism, Mental Health and
behaviour described as Challenging.
Individuals use our outreach services for specific things such as shopping, using public
transport, attending social and community clubs, doing activities such as swimming, going
to work or going to college. Wherever possible the same support worker attends each time.
